The California Academy of Family Physicians Foundation will undertake virtual diabetes collaboratives and collaboratives modeled after the Institute for Healthcare Improvement's Breakthroughs in Chronic Illness Care. Supported by The Physicians Foundation, the California Academy of Family Physicians Foundation will recruit practice teams to participate in a virtual collaborative learning experience that blends online learning with face-to-face educational experiences; additional teams of primary care physicians will be recruited for participation in collaboratives in underserved counties, utilizing only face-to-face educational experiences. All teams will commit to an aggressive practice improvement and change adoption methodology that draws on established models for providing high quality chronic illness care and well-documented methods of establishing practice improvements—with a focus on the small practice environment.
